The placement of the mammary implants behind the pectoral muscles is being performed more frequently each day.  The indications followed with the patients who visit us in our office are the following: 

1.- The presence of a tumor or tumors in the mammary gland

2.- Severe mammary hypoplasy

3.-Family history of breast cancer

4.- History of breast cancer in the opposite mammary gland

5.- Unfounded fear of developing breast cancer in the future because of the implants

6.- Previous history of a mammary abscess 

In these very specific cases, I normally place the implants in a retromuscular position, but now these indications have extended and even more plastic surgeons in the world are using this technique as a standard in all their patients, with the modification of placing the implant under the gland in the inferior pole and under the muscle in the superior pole.

The outcome is a stylized breast with less possibilities of developing a capsule and therefore more natural. The routine exams for detecting cancer by images are easier. 

Still the only contradiction considered are patients who practice sports. 

The decision of the use of implants in a retromuscular or retroglandular  position,  will be made according  to the surgeon’s experience.
